Conduct informal one-on-one discussions that the manager and the employee have talked about during their regular check-ins throughout the year. Whatever the frequency of these check-ins, the table can be used by adding a new row for every conversation, thereby always keeping track of previous agreements or discussions. The questions are purposely kept very open and informal, so that they can cover all manner of topics.
